The care for sickle cell disease (SCD), a serious genetic blood disorder affecting approximately 1000 individuals in Washington, DC, remains fragmented. The patient centered medical home model has been demonstrated to have tremendous benefits in improving quality of care, health outcomes, family and patient satisfaction, and decreasing health care costs for chronic illnesses. We have established a “one stop shopping” model of comprehensive care for children with SCD that includes care coordination, navigation and continuous primary and specialty care. The DC GAPS staff are leaders in the field of SCD and have the experience to make this a model demonstration project.
